  • Information on procedure concerning the sending of the ashes of a deceased person from Australia to Poland

     

    The Consulate requires the following documents in order to issue a certificate allowing the entry of the ashes to Poland:

     

    -    death certificate with Apostille (original),

     

    -    cremation certificate (original),

     

    -    certification of ash box contents (original),

     

    -    burial permission issued by local authorities in Poland (such a document is usually arranged by the     family of the deceased person - a copy of the document sent to the Consulate by fax is sufficient).

     

    Please note that the Consulate requires the burial permission issued by STAROSTA or PREZYDENT MIASTA. The certificate issued by a priest or a local parish will not be accepted.

     

    Please note that death certificate should be stamped with APOSTILLE by Department of Foreign Affairs and Trade.

    The consular fee is 81 AUD (paid by bank cheque,  money order or cash).

     

    Please note that the cremated remains should be placed in a sealed urn bearing the cremation number and the name and surname of the deceased. The consular seal is no longer required.

     

     

    Information on procedure to issue a certificate for transportation of a body to Poland

     

    The Consulate requires the following documents in order to issue a certificate for transportation of a body to Poland:

     

    -    death certificate (original),

     

    -     embalming certificate (original) – preparation of the body for transportation (original),

     

    -     certification of coffin contents (original), stating that the body was placed in a sealed metal coffin and that the coffin contains only the body of the deceased,

     

    -     burial permission issued by local authorities in Poland (office of the starosta); such a document is  usually arranged by the family of the deceased person (a copy of the document sent to the Consulate by fax is sufficient).
